Duties

Join our multidisciplinary Community Mental Health & Wellness team at Wilder in providing clinical service delivery that supports our community's mental wellness, critical to Wilder's mission of improving lives today and for generations to come! We are hiring a full-time Care Coordinator to take part in our interdisciplinary care team. You will provide coordination of services and resource support to our clients in the community to get people what they need when they need it. They will coordinate all incoming client referrals in the Community Mental Health Outpatient programs at Wilder Center, complete intake process, opening and closing of clients, and scheduling. Work with clients and families to ensure high satisfaction with program services. Assess for resource deficits and connect clients with community partners to fill deficits in social determinants of health. Complete communications and support tasks in an efficient and quality conscious manner. Work in a coordinated way with community partners, providers, clients and their families to support the full range of health needs including mental health and physical health.
If hired as Bilingual Staff - Interact with clients by speaking English and one or more additional language, e.g., Hmong, Vietnamese, Khmer or Spanish. Explain documents/information to clients with marginal English language skills, as needed.

Here is the pay breakdown depending on the qualifications of the applicant
LGSW = $27-28/hour
LPN = $28-30/hour
En route to bachelors or bachelor's degree = $23/hour
Additional pay for fluency in second language (Spanish, Hmong, Karen)
Job Qualifications
Minimum of 2 years' experience in social service/mental health field and experience required.
Experience working directly with clients/patients as a health care coordination, case management, or referral tracking preferred.
Experience working in a social service or health care setting preferred.
Education: AA minimum or BA.
Training: LGSW or LPN highly desirable, but not required if there's experience.
If hired bilingual - fluency in speaking and proficiency in writing English language and fluency in speaking one or more additional languages, e.g., Hmong, Vietnamese, Khmer, Karen, Spanish, etc.
